This document defines editorial principles to govern the development of standards for the structure and content of health records. The principles are needed to ensure that the future standardisation of paper and electronic health records (EHRs) – across professions, disciplines and specialties – properly reflects good practice and supports the growth of high quality, accurate, trustworthy and useful documentation about the health, healthcare and welfare of individuals.
