Record standards safety case

The standards for patient records developed have undergone a formal risk assessment process to ensure clinical safety. At each step of the consultations to develop standards – be they workshops, meetings, national surveys – hazards were identified, reviewed and mitigations/actions considered. Nevertheless, some risk is inherent in the standards but most has been:(a) mitigated by the development of the standards or(b) the residual risk has been transferred (with guidance) to the implementers.The Hazard Log provides guidance for system developers and implementers. It is important that this guidance in relation to these hazards become requirements for implementation.
