A collection of shared resources from the Future Hospital: Journey towards integration event that took place on 23 June 2017, which brought together a diverse group of clinicians, patients and commissioners to explore the barriers and facilitators to developing integrated services for long-term conditions.
The aims of the event were to:
- present perspectives on integration from patients, primary and secondary care, and commissioners
- share experience from the Future Hospital research project in Oxfordshire which is piloting the integration of diabetes services, and provide a forum for delegate to share their own experience
- explore key issues and solutions in the areas of IT integration, governance and finance, patient engagement, clinical engagement and training.
